Since 2019, MCHN has served as a trusted community-based partner in Allina Health’s Integrated Health Partnership (IHP)—a program administered by the Minnesota Department of Human Services. The IHP initiative is designed to enhance the quality of care for individuals enrolled in public health care programs, with a focus on improving outcomes and promoting long-term wellness.
Allina Health’s IHP prioritizes Medicaid patients facing serious mental illness, substance use disorders, and other complex health and social challenges. Through this collaboration, Allina refers patients to MCHN for personalized case management services that foster stability, recovery, and community integration.
Building on the success of this partnership, the Northwest Metro Alliance—an affiliation between Allina Health and HealthPartners—welcomed MCHN into its IHP network in 2022, following strong results from pilot programs and demonstrated impact within the Allina IHP.
All services are tailored to the individual’s unique needs and recovery goals.
Allina Health care guides send referrals to a centralized MCHN “hub.”
The hub assigns the referral to one of three MCHN agency case management teams.
MCHN case managers work with individuals for 60–100 days, providing access to critical resources.
